Est / proposal sheets

I want to have some custom est / propsal sheets made up and I was wondering if anyone uses any place online that they could recommend. (Or local chain stores)

I just need a two part form... When I do the job I bring an invoice printed out from QB..

Dont know of any online stuff but, I did see a local glass shops invoices the other day and on the invoice was car glass layed out in the correct order ex. windshield in front then left glass on left and right glass on right then backglass. For residential commecial jobs have an area where you can put note and or freehands of the windows.
