Invoices and Disclaimers

Does anybody have a copy of the disclaimer they put on their websites or on the backs of their invoices? Also, now when I do a job I dont have any form of paperwork besides i hand out a few business cards a decal and a care sheet. I do not keep any kind of invoice and I would like to but can't find a template or any ideas anywhere, does anybody have one on here? I heard 44tools has some invoices but I cannot get on their website for some reason. So anybody who has anything on here that I could work with would be appreciated!
